New Student Registration – What to Bring

Summer Registration for the '08 - '09 School Year:
June 17th  4pm-7pm, June 18th 4pm-7pm, June 21st 8am-10am

Please bring the following items with you to register a student:

 
1. Birth certificate, passport if it lists the parents, or other acceptable document of identity.
2. Current Driver’s License for the person registering the student.  The person registering the student must be the parent listed on the Birth Certificate or Passport or have an Application of Determination showing custody. (This is only obtainable at the HISD Administration Building on W. 18th St .)
3. Divorce decree (if applicable) to verify custodial rights.
4. Social Security card (optional)
5. Proof of residence
        a. Apartment dwellers:  an original lease agreement (no copies), signed by  
            management, with the student listed as an occupant is required.
        b. Home dwellers:  A current (within the last 2 months) utility bill with the
            parent’s name and address.
        c. Non-zoned Students:  a transfer letter showing that you have been granted
            a transfer from HISD to attend Westside.
6. Immunization records (Please be advised that students who are not in compliance with immunization requirements, by law, do not meet attendance requirements.)

DPT/DT - 3 doses required with last dose after age 4 and 1 within 10 years.
Polio (oral) 4 doses required, with last dose after age 4
MMR (rubella, rubeola, mumps) 2 doses required, 1st dose after age 1 
Hepatitis B - 3 doses required with last dose given anytime
Varicella (Chickenpox) 1 dose on or after 1st birthday or history of illness

7. Final report card with promotion status from this past school year is required of incoming 9th graders.
8. If the student will be a 10th-12th grader, a copy of an unofficial transcript is necessary. If the unofficial transcript is not available at registration, the student will be placed into 9th grade until the transcript can be produced.
9. Special Education papers/ARD records if applicable.
10.  Withdrawal papers from the previous school are required for registration during the school year.
