Enrollment requirements:

1.     Parent or legal guardian (as appointed by the courts) must accompany the student.  Legal documents must be provided in cases involving divorce and/or legal guardianship.  Parents will be asked to show their driver’s license or valid proof of identification.

2.     Proof of address of parent or court-appointed guardian

• If living in a house, the most recent light bill

• If living in an apartment, a letter from the apartment manager on apartment stationery.  A lease is NOT sufficient.  This letter should state whom the student lives with and in what apartment.  It must be signed by the apartment manager and should include a contact phone number.

3.     Proof of age/identity

•  Birth certificate – especially if you are new to HISD

•  Social security card if student has an assigned number

•  Passport with Visa ONLY if the student has never enrolled in a school in the United States.

4.     Health Records – required shots

• Diphtheria – tetanus (DT Booster) in the last ten years

•  Polio Booster since the age of four

• Measles, mumps, rubella (two doses)

• Hepatitis shot series suggested

• TB skin test

5.     School Records from your last school

• Report card from the previous semester

• Unofficial transcript – must provide a translation if in another language and a grading scale

• Withdrawal form from all Texas schools if registering within the school year
