Students at Michael DeBakey HSHP
must be in compliance with both state and district guidelines, as well as
clinical rotation site policies. Clinical rotation,( 11th and 12
th grade) ,sites require
Hepatitis B (series of three vaccinations),
TB skin test (given within 1 year),
Varicella (Chicken Pox) vaccines be current. (If a student has had the Chicken Pox, a written statement from a physician or student's parent or guardian verifying the approximate date the student had the disease, is considered reliable information.)
PLEASE SAVE ALL YOUR ORIGINAL IMMUNIZATION DOCUMENTS AS THEY ARE REQUIRED BY SCHOOLS AND UNIVERSITIES IN THE FUTURE.
Please bring all new immunization information to the school nurse to update your child’s records.
